📝 Step-by-Step Guide to Initial Registration

1. Wait for the Official Results

Visit the PRC website to confirm your name on the official list of passers once the results are released.

2. Prepare the Required Documents

Duly accomplished Oath Form (Panunumpa ng Propesyonal)
Two (2) passport-size ID photos with name tag on white background
Community Tax Certificate (Cedula)
Photocopy of valid government-issued ID
Payment for registration fee (₱1,050 approx.)

3. Set Your Online Appointment via PRC LERIS

Go to https://online.prc.gov.ph
Create an account or log in
Choose Initial Registration under transactions
Select “Nursing” and your preferred PRC office
Download and print your Oath Form and Appointment Slip

4. Report to Your PRC Office

Wear proper attire (no sleeveless tops, shorts, or slippers)
Submit all required documents
Sign the registry and affix thumb mark
Take the Oath of Professionalism

5. Claim Your PRC ID and Certificate

Your PRC ID and Certificate of Registration will be released on your indicated claim date.
