📌 Definition

Celiac Disease is a chronic, autoimmune disorder in which ingestion of gluten (a protein in wheat, rye, and barley) triggers immune-mediated inflammation and damage to the small intestinal mucosa, resulting in malabsorption.

🧠 Anatomy & Physiology

  • Small intestine: Site of nutrient absorption; lined with villi and microvilli to maximize surface area.
  • In celiac disease, immune response to gluten damages villi, leading to villous atrophy and decreased absorption.
  • Results in malabsorption of nutrients, vitamins, and minerals.

⚠️ Etiology: Causes & Risk Factors

  • Genetic predisposition: HLA-DQ2 or HLA-DQ8 genes.
  • Environmental: Exposure to gluten-containing foods.
  • Other autoimmune diseases (type 1 diabetes, thyroiditis) increase risk.
  • Family history of celiac disease.

  • Gastrointestinal: Chronic diarrhea, steatorrhea (fatty stools), bloating, abdominal pain, flatulence, weight loss.

  1. Ingestion of gluten triggers abnormal immune response in susceptible individuals.
  2. Autoimmune inflammation targets small intestine villi (especially proximal intestine).
  3. Villous atrophy, crypt hyperplasia, and lymphocytic infiltration develop.
  4. Impaired absorption of nutrients, vitamins, minerals; complications of malnutrition ensue.

  • Genetic testing: HLA-DQ2/DQ8 in unclear cases.

  • Strict lifelong gluten-free diet: Only effective treatment—removes all sources of wheat, rye, and barley.
